I would like to nominate Sidra for LotW for running out of game time mid-instance in Utgarde Pinnacle.  One minute the healer was there, the next minute she's gone.  (This was after her husband asked about whether she needed to renew it.) 

Xaeth also gets another award for backing off ledge and falling to his death during a fight in Icecrown instance.
